2009 Annual Enrollment Overview
Annual Enrollment for 2009 is here and we have important information about the process to help you select your healthcare benefits!
If you are currently enrolled in the Plan, paper enrollment forms and Group Medical PPO Plan information were mailed out the week of October 13.
If you are not currently enrolled in the State Farm Group Medical PPO Plan, you will need to contact the Human Resources Services Center (HRSC) at 1-877-272-1999 to request a late enrollee form.
Compressed enrollment period
2009 Annual Enrollment will run from October 13 – November 26, 2008. Enrollment forms are due November 11. November 26 will be the FINAL cut-off date for any changes.
Non-required enrollment
Benefit elections from 2008 will be carried over into 2009 for retirees who do not make any changes. Remember, you only need to return the enrollment form if you make changes to your medical coverage. If you do make a change, you will receive a confirmation statement reflecting those changes.
"Do the math" – it could save you money
Although 2009 enrollment is non-required, you will still want to review the Annual Enrollment information and consider whether you need to make any changes given your needs. For example, plan rates are likely to change on an annual basis, and you may want to consider the monthly contribution savings that can be realized from moving to an option with a higher deductible. Larger deductibles can be intimidating; however, if you saved the monthly contribution savings from choosing a higher deductible, you might find you would be financially better served with a higher deductible.
This is also a good time to consider the advantage of using the $1,000 preventive services benefit available to Group Medical PPO Plan participants. This valuable benefit is not subject to the annual deductible, and can be used to cover eligible preventive services. Examples of preventive diagnostic tests and services include but are not limited to screening mammography, serum cholesterol test, resting electrocardiogram (EKG), tetanus and influenza immunizations, cervical smear or Pap smear test, fecal occult blood test, prostate specific antigen test (PSA), and bone mineral density screening.
