2007 Annual Enrollment Overview

Benefits enrollment for 2007 is right around the corner. This communication contains important information about the process to help you prepare for 2007 Annual Enrollment (AE). Paper enrollment forms and benefit plan information will be mailed to you the week of October 16, 2006 if you are currently enrolled in the State Farm Group Medical PPO Plan.

If you are not currently enrolled in the State Farm Group Medical PPO Plan, you will need to contact the Associate Services Center (ASC) at
1-877-272-1999 to request a late enrollee form.


Compressed Enrollment Period

2007 annual enrollment will run from October 16 – November 30, 2006.

This year, associates and retirees will no longer have until the end of December to make or change enrollment elections. Paper enrollment forms are due November 10 and November 30 will be the FINAL cut-off date for any changes.


Non-required Enrollment

Benefit elections from 2006 will be carried over into 2007 for retirees who do not make any changes. If you make changes to your medical coverage, you will receive a confirmation statement reflecting those changes.
