The Centers for Medicare & Medicaid Services (CMS) require State Farm to provide all Medicare–eligible participants with a Notice of Creditable Coverage as it relates to their Group Medical PPO Plan prescription drug coverage. If you are a Medicare–eligible individual either enrolled in, or eligible to enroll in, one of the options under the State Farm Group Medical PPO Plan, you should receive this Notice.

The intent of the Notice is to advise you the Prescription Drug coverage under the State Farm Group Medical PPO Plan is at least as good as or better than Medicare's basic drug benefit (Part D). This means the State Farm coverage is “creditable” and therefore, you can join a Medicare Part D plan later without penalty.
