The Centers for Medicare & Medicaid Services (CMS) require State Farm to provide all Medicareeligible participants with a Notice of Creditable Coverage as it relates to their Group Medical PPO Plan prescription drug coverage. If you are a Medicareeligible individual either enrolled in, or eligible to enroll in, one of the options under the State Farm Group Medical PPO Plan, you should receive this Notice.
The intent of the Notice is to advise you the Prescription Drug coverage under the State Farm Group Medical PPO Plan is at least as good as or better than Medicare's basic drug benefit (Part D). This means the State Farm coverage is creditable and therefore, you can join a Medicare Part D plan later without penalty.
NOTE: If you are a Medicare-eligible State Farm retiree or eligible dependent, and you need to request a copy of this Notice specific to you, please call the State Farm Benefits Center at 1-866-935-4015.
