In most locations, employees can choose between the State Farm Group Medical Preferred Provider Organization (PPO) and a Health Maintenance Organization (HMO). A variety of resources are provided to help you choose the health care option that best meets your needs.
The plan provides coverage for preventive care, dental restorations, and orthodontia care.
The plan provides comprehensive vision benefits, including routine eye exams and contact lenses or glasses.
Health Care and Dependent Care Flexible Spending Accounts (FSAs)
You can choose to allocate before-tax dollars to pay eligible medical and/or dependent-care expenses. Participating employees don’t pay federal or state income taxes (in most states) or Social Security taxes on contributions to, or distributions from, these accounts.
You can choose to participate in State Farms Commuter Benefits program for US Employees. This program provides eligible employees an opportunity to use pre-tax dollars to pay for certain transportation expenses on a monthly basis. Participating employees do not pay federal income or state income taxes (in most states), or Social Security taxes on qualifying expenses. In addition to the tax savings, this program offers the convenience of online ordering and home delivery of transit/parking passes or vouchers.
Provides basic life and accidental death and dismemberment (AD&D) protection, with a coverage amount that is two-times your salary. State Farm pays a portion of the premium. Additional voluntary AD&D coverage is also available.
Provides income during prolonged disability due to an illness or injury.
Voluntary Accidental Death & Dismemberment (AD&D)
Provides protection against the loss of life or certain bodily injuries resulting from covered accidents. This coverage provides financial protection for accidents sustained on or off the job, anywhere in the world, in which accidental death, loss of sight, loss of use, and/or dismemberment occurs. The plan offers coverage for you and your eligible dependents. In addition, the Voluntary AD&D Plan also covers you (the employee only) for permanent and total disability resulting from an accident.
