- date and location of the occurrence
- insurance card
- insurance information and phone number of other person
- general description of what happened
- police department involved and report number
Life Insurance
When reporting a claim, have the following information handy:
- Insured's full name
- Date of birth
- State of residence
- Policy number(s)
- Date of death
- Cause of death
