When reporting a claim, have the following information handy:

  • date and location of the occurrence
  • insurance card
  • insurance information and phone number of other person
  • general description of what happened
  • police department involved and report number


Report a life insurance claim


When reporting a Life claim, have the following information handy:

  • Insured's full name
  • Date of birth
  • State of residence
  • Policy number(s)
  • Date of death
  • Cause of death
