How do I report a death?

There are three methods to report the death of a person who is insured by a State Farm life policy or annuity. Please select one of the following to start the life claims process.

  • Contact an Agent -- Search for a State Farm agent. Although you may report a death claim to any agent, the agent of record will contact the beneficiary designated in the policy.
  • Contact a Life Claims Office -- Search for the toll-free number of a Life Claims Office.
  • Report online -- Submit a form to begin the life claims process.
