Prescription Drug (Part D) Plans

Looking for information on Medicare Health Benefit Options?

First, it's important for you to know that you have many plan options to choose from. You can enroll in traditional Medicare or select from a variety of other Medicare Advantage health plans that include prescription drug coverage and other benefits.

State Farm® has joined with Humana® in an alliance relationship to market Medicare Prescription Drug (Part D) Plans.

The following information given to State Farm by Humana provides an overview of these plans. Please call a State Farm agent for more detailed information.

Prescription Drug (Part D) Plans

Protection from out-of-pocket costs

When you choose one of Humana's Medicare Prescription Drug Plans, you receive guidance to help you understand how the Medicare Part D plan works and how you can save money on your prescriptions. Plus, Humana makes it easy for you to keep track of your prescription drug costs. Each month, you will receive our SmartSummary RxSM statement that shows what prescriptions you bought that month and how much you spent.

Different plans for different needs

Humana offers three stand-alone Medicare Prescription Drug Plans (PDPs) to help you balance your health needs and budget. Remember, "stand-alone" means they're separate from your Medicare coverage. Stand-alone PDPs let you add drug coverage to your Original Medicare or your additional Medicare Supplement plan.

Humana PDP Standard

A basic plan, equal to the federal government's minimum requirements for coverage.

Humana PDP Enhanced

No deductible and broad coverage with co-payments to help you manage your costs.

Humana PDP Complete

No deductibles and low co-payments to help save you money AND it covers the balance of Preferred Generic drug costs (after co-payments) while in the coverage gap.

A Medicare-approved Prescription Drug Plan available to anyone entitled to Part A and/or enrolled in Part B of Medicare through age or disability who continues to pay Medicare-applicable premiums. Co-payment, service area and benefit limitations apply.

State Farm agents will not offer Humana Medicare Part D in Massachusetts, New Jersey or Rhode Island.
