Requesting a Change to a Life Insurance or Annuity Policy


If you are considering changing your life insurance or annuity policy, we strongly urge that you first contact your State Farm agent, who will be happy to assist and advise you.

If, for any reason, your State Farm agent is unavailable, you may initiate a change by completing, printing, signing (if required), and mailing the appropriate Request Letter below. (Adobe Acrobat .pdf format)

Please do not submit your policy with the Request Letter.

For security purposes, we are unable to accept Request Letters sent via e-mail.
