Updating address/phone number

• By phone
  Call 1-800-447-4930. All redemption proceeds will be sent to your former address during the fifteen day period after we receive your address change request unless the redemption request is in writing and accompanied by a signature guarantee for each registered owner.

  Mail your address or phone number change to State Farm Mutual Funds®, P.O. Box 219548, Kansas City, MO 64121-9548. A registered account owner's signature is required to change an address by mail. If you request an address change, redemption proceeds will be sent to the former address during the fifteen day period after the
  Manager receives your request unless the redemption request is in writing and accompanied by a signature guarantee for each registered owner.

Updating beneficiary information

Call 1-800-447-4930 and request a Designation of Transfer on Death Beneficiary Form (non-retirement accounts). For retirement accounts, you should request a Designation of Beneficiary Form, and for Coverdell Education Savings Accounts you should request a Designation of Death Beneficiary Form. These forms can also be downloaded from literature & forms.

Updating bank information

You can change bank instructions by mail, fax (1-816-471-4832) or phone (1-800-447-4930) as long as the bank account and mutual fund account owner(s) are identical. If the bank account and mutual fund account owner(s) are different, mail a written request (not by fax) signed by each shareholder and with at least one bank account owner's signature guaranteed to:

Updating privileges on my account

Call 1-800-447-4930 and request an Investor Account Services Form or download it from literature & forms. This form allows you to establish or change multiple services for your account. These services include automatic investments, capital gains and dividend distribution options, systematic withdrawal, and telephone/online privileges. Some services require a signature guarantee.

Updating names on an account

For instructions on removing your name from an account, please call 1-800-447-4930.

To add your name as a joint owner to an account, call 1-800-447-4930 and request an account application. All owners of the account must sign the new application. A signature guarantee is required (need both old and new signatures) and can be faxed to (816) 471-4832.

To change your name on an account, mail a signed letter stating your name change to State Farm Mutual Funds, PO Box 219548, Kansas City, MO 64121-9548. Be sure to list all of the account numbers to which the name change applies.

Updating a Social Security Number or Taxpayer Identification Number

A W-9 needs to be completed to provide us with your correct Social Security Number or Taxpayer Identification Number. Call 1-800-447-4930 and request this form or download it from literature & forms.
