- Non-Tax Qualified Transfer/Liquidation Request Form (PDF 46 KB)
- Used to liquidate funds from a non-tax qualified account to purchase shares of State Farm Mutual Funds® Account
- Should not be used to transfer Retirement accounts
- Separate form must be completed for each account you wish to transfer /Liquidate.
- Investor Account Services Form (PDF 199 KB)
- Used to establish or change services for your account.
- Account services include address changes, automatic investments, capital gain and dividend distribution options, systematic withdrawals, and telephone/Internet privileges.
- Designation of Transfer on Death (TOD) Beneficiary Form (PDF 37 KB)
- Used to designate a transfer on death beneficiary on your mutual fund account (Not used for IRAs – see IRA Section)
- Corporate/Organization Resolution Form (PDF 116 KB)
- Used to name individuals authorized to act on behalf of your corporation, organization or partnership.

- Used to provide us with your correct Social Security Number or Taxpayer Identification Number.

- Used to request a one-time distribution or to establish a systematic withdrawal plan for a Traditional IRA, Roth IRA, SEP IRA, SIMPLE IRA, or 403(b) due to the death of a participant.

What this means for you: When you open an account, we will ask you for your name, address, date of birth, and other information that will allow us to identify you. We may also ask to see your driver's license or other identifying documents.
