Medical

In many locations, employees can choose between the State Farm Group Medical Preferred Provider Organization (PPO) Plan and an Insured Medical Plan option (Health Maintenance Organizations [HMOs] and Accountable Care Organizations [ACOs]). A variety of resources are provided to help you choose the employee health care option that best meets your needs.

Dental

The plan provides coverage for preventive care, dental restorations and orthodontia care.

Vision

The plan provides comprehensive vision benefits, including routine eye exams and contact lenses or glasses.

Health Care and Dependent Care Flexible Spending Accounts (FSAs)

You can choose to allocate before-tax dollars to pay eligible medical and/or dependent-care expenses. Participating employees don't pay federal or state income taxes (in most states) or Social Security taxes on contributions to, or distributions from, these accounts.

Commuter Benefits

You also have the option of participating in the employee commuter benefits program. This program provides eligible employees an opportunity to use pre-tax dollars to pay for certain transportation expenses on a monthly basis. Participating employees do not pay federal income or state income taxes (in most states), or Social Security taxes on qualifying expenses. In addition to the tax savings, this program offers the convenience of online ordering and home delivery of transit/parking passes or vouchers.

Life Insurance

Group Life Insurance provides basic life and accidental death and dismemberment (AD&D) protection, with a life insurance coverage amount that is two-times your annual base salary. State Farm pays a portion of the premium. Voluntary AD&D coverage is also provided up to $100,000 in coverage. The plan also offers coverage for you and your eligible dependents.

Long-Term Disability

Provides income during prolonged disability due to an illness or injury.

Short Term Disability

The Short Term Disability policy provides salary continuation for employees who have approved medical absences, such as recovering from childbirth, surgery, accident or illness lasting more than seven calendar days.

Voluntary Accidental Death & Dismemberment (AD&D)

This employee benefit provides protection against the loss of life or certain bodily injuries resulting from covered accidents. This coverage provides financial protection for accidents sustained on or off the job, anywhere in the world, in which accidental death, loss of sight, loss of use and/or dismemberment occurs. The plan also offers coverage for you and your eligible dependents.
