Health & Welfare: Group Vision

As part of your Total Rewards package, State Farm offers Group Vision coverage for you and your eligible dependents. There are two options to choose from: Base Choice Plan and Premium Choice Plan. Both vision plan options are administered by Vision Service Plan (VSP) and cover the following:

  • Eye exam by a VSP provider every calendar year.
  • Both plans provide lens options that include single vision, lined bifocal or lined trifocal lenses, in regular plastic, glass or polycarbonate lenses every calendar year. Progressive lenses have a fixed copayment of $55. The Premium Choice Plan offers an anti-reflective coating for $40.
  • Other lens options are available at discounted rates.
  • New frames of your choice (every other calendar year) up to either $120 or $200, plus 20 percent off any amount over the allowance.
  • Contact lens care each calendar year. Choose contacts instead of glasses, for $120 or $200 allowance to the cost of your contacts and the contact lens exam, including fitting and evaluation. This exam is in addition to your vision exam to ensure proper fit of contacts.

General Information

While it's most cost-effective for you to use an in-network provider, you have the option to use a provider outside of the VSP network as well. However, if you choose a provider outside of the VSP network, you will be responsible for any copayments that apply. You will also need to submit your claim to VSP for partial reimbursement. When using an out-of-network provider, you may receive a lesser benefit and pay more out-of-pocket.

Enrollment Details

Remember, enrollment in any State Farm plan remains optional. However, it's important you understand that if you fail to enroll or decide to waive or cancel vision coverage, you must wait until the next Annual Enrollment Period to enroll. Your coverage will be effective the following January 1. If you have a qualified Health Insurance Portability and Accountability Act (HIPAA) special enrollment event (e.g., loss of coverage, obtaining a new dependent due to marriage, a birth, an adoption or placement for adoption), you may be eligible to enroll in medical coverage mid-year. You must notify the State Farm Benefits Center within 31 days of an event in order to be eligible for coverage.
