Good Neighbor Healthy Living
The Good Neighbor Healthy Living Program is available to retirees under 65 years of age who are covered under the State Farm® Group Medical PPO plan. This includes spouses and dependents who are also covered under the PPO plan.
Good Neighbor Healthy Living, administered by Alere®, is a system of coordinated healthcare interventions and communications for covered individuals with specific chronic health conditions in which patient self-care efforts are significant factors for improving health status.
The health conditions managed by Good Neighbor Healthy Living are:
- Chronic Obstructive Pulmonary Disease (COPD)
- Coronary Artery Disease (CAD)
- Heart Failure
- Musculoskeletal Pain
This program provides confidential, proactive, and supportive assistance through an education-based program. Participation in the program is completely voluntary. If you or your spouse has one of these conditions and would like to know more about the program, simply call 800-698-8546 and a nurse will be available to assist you further.
You can also access Good Neighbor Healthy Living online at www.SFLiveWell.com. If it is your first time to the Web site, you will need to register and develop a profile. Follow the steps provided for "First Time Here".
