If you are unable to log in or haven’t created an account, locate the paper copy of your declarations.

Look for your form numbers under a section either called “forms,” “endorsements” or “options.”

If logged in to statefarm.com

1. On the My Accounts page, select your type of insurance policy.
2. Scroll down to the title “My policy details.”
3. In the “Policy documents” section, look for your form number.
