*Plan N pays 100% of the Part B coinsurance, except for a copayment of up to $20 for some office visits and up to a $50 copayment for emergency room visits that don’t result in an inpatient admission.

 

Exclusions, exceptions, and limitations

The following expenses are not eligible to be included in the calculation of benefits under this policy:

  1. Expenses that are not eligible to be included in the calculation of benefits payable under Medicare;
  2. Expenses incurred for a confinement in a hospital or skilled nursing facility beginning before the insured's effective date of coverage;
  3. Charges which exceed the Medicare Eligible Expense as specified in the Benefit provisions; or
  4. Expenses incurred while this policy is not in force subject to the Extension and Limitation of Coverage provision.
