Exclusions, Exceptions, and Limitations
The following expenses are not eligible to be included in the calculation of benefits under this policy:
- Expenses that are not eligible to be included in the calculation of benefits payable under Medicare;
- Expenses incurred for a confinement in a hospital or skilled nursing facility beginning before the insured's effective date of coverage;
- Charges which exceed the Medicare Eligible Expense as specified in the Benefit provisions; or
- Expenses incurred while this policy is not in force subject to the Extension and Limitation of Coverage provision.
