Copyright or intellectual property infringement procedures

We respect the intellectual property of others. If you believe that a work has been copied in a way that constitutes copyright infringement or its intellectual property rights have been otherwise violated, please provide Notice to our Intellectual Property Agent with the following information:

  1. A description of the copyrighted work or other intellectual property that you claim has been infringed or violated, include a copy of the copyrighted work and the name and contact information of the owner of the work;
  2. A description of the material or content that you claim is infringing, include a copy of the material/content and where it is located on the State Farm website;
  3. Your address, telephone number, and email address, and a statement on your relationship with the owner of the copyrighted work or other intellectual property that you claim has been infringed;
  4. A statement by you that you have a good faith belief that the disputed use is not authorized by the copyright owner, its agent, or the law
  5. A statement by you, made under penalty of perjury, that the above information in your Notice is accurate and that you are the copyright or intellectual property owner or authorized to act on the copyright or intellectual property owner's behalf; and
  6. Your electronic or physical signature affirming the information and statements made in the Notice.
