Where to find your declarations page

Your declarations page is an indexed summary of your State Farm® policy. It includes important information like when the policy begins, ends and what it includes.

Find it in the Document Center that you can access by logging in to your account. It also may be called a “policy notice.” If you don’t see either title, you can use a renewal notice instead to access it.

What’s included in an insurance declarations page?

Among the inventory included in your auto policy, for example, could be:

  • Policy number, length and effective date, premium amount, and deductibles
  • Main policyholder and other named insureds
  • Listed drivers and any excluded one
  • Year, make, model and VIN of all vehicles on the policy

Any time you think you might need to file a claim, consider checking your policy. Understanding what your policy covers could help you avoid surprises when you file a claim.

Is a declarations page proof of insurance?

Yes, a declarations page can serve as proof of coverage. Some mortgage lenders ask to see proof of insurance once a year, and your declarations page should suffice. You may also need to present it if you’re refinancing your home, taking out an equity loan or want to add additional endorsements from another insurer.
